Job Description

Simbioza Inc. is seeking a Patient Navigator to support patients across Florida as they move through their healthcare journey. This is a part-time, telecommute / work-from-home position supporting patients remotely.

This role is ideal for an experienced navigator who understands Florida healthcare systems and enjoys helping patients overcome barriers to care, especially related to Medicare and Medicaid.

You will work directly with patients, providers, and care teams to ensure patients understand their options, stay on track with care, and feel supported every step of the way.

What You’ll Do Day-to-Day

  • Guide patients through appointments, referrals, and follow-up care

  • Support patients with Medicare and Medicaid eligibility, benefits, and coverage questions

  • Help patients understand next steps in clear, simple language

  • Coordinate with clinics, providers, and community resources

  • Identify and resolve barriers to care such as insurance issues or scheduling challenges

  • Maintain accurate documentation and patient records in EHR systems

  • Communicate with patients by phone, email, and telehealth

  • Protect patient privacy and ensure HIPAA compliance at all times

Required Qualifications

  • Active Florida state certification as a Patient Navigator

  • Minimum 1 year of hands-on experience working as a Patient Navigator

  • Strong working knowledge of Medicare and Medicaid programs

  • High school diploma or GED

  • Experience using EHR systems and scheduling tools

  • Strong communication and organizational skills

  • Ability to work independently in a remote environment

  • Must currently reside in Florida and be authorized to work in the U.S.

For a Florida-certified Patient Navigator role, residency in Florida is the norm and is required.

Preferred (Nice to Have)

  • College coursework or degree in a health-related field

  • Experience in clinics, hospitals, care coordination, or case management

Schedule & Compensation

  • Part-time, hourly position

  • Flexible schedule

  • Hourly rate based on experience and certification

  • Level II background check required

  • This is a telecommute / work-from-home position supporting patients in Florida
