Patient Navigator
Job Description
Simbioza Inc. is seeking a Patient Navigator to support patients across Florida with care coordination, follow-ups, and healthcare system navigation. This is a part-time, telecommute / work-from-home position for candidates who reside in Florida and have completed recognized patient navigation or care coordination training.
This role works closely with patients and care teams to improve access to care, reduce barriers, and support patients enrolled in Medicare and Medicaid programs. The Patient Navigator operates under the supervision of a Nurse Practitioner and supports Medicare-billable care management activities.
Key Responsibilities
Support patients with care coordination, referrals, and follow-up needs
Assist with Medicare and Medicaid eligibility, benefits, and navigation questions
Conduct patient outreach and ongoing follow-ups
Provide clear education to patients regarding care plans and next steps
Coordinate with providers, clinics, and community resources
Document care coordination activities in the EHR
Communicate with patients via phone, email, and telehealth
Maintain HIPAA compliance and patient confidentiality
Required Qualifications

Completion of a recognized patient navigation or care coordination training program, such as:
Healthcare Navigator Specialist Certificate (e.g., Eastern Florida State College or equivalent), or
Susan G. Komen Navigation Nation training, or
Comparable formal navigation or care coordination education
At least 1 year of experience in patient navigation, care coordination, or a related healthcare role
Working knowledge of Medicare and Medicaid
High school diploma or GED
Comfort using EHR systems and digital communication tools
Strong organizational and communication skills
Ability to work independently in a remote environment
Must reside in Florida and be authorized to work in the United States
Preferred Qualifications
Experience supporting NP-led or physician-led care teams
Experience working with vulnerable or underserved patient populations
